Qualifications

• Registered Nurse license in WI or eligibility to practice in Wisconsin.
• BLS for Healthcare Providers certification preferred or required on hire.
• Current knowledge of hospice and end-of-life care regulations.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients across hospice settings.
• Educate patients and families on disease trajectory, management, and end-of-life care.
• Review caseload, ensure appropriate level of care, and refer to team members as needed.
• Lead the IDG for caseload and develop/implement Plans of Care with interdisciplinary input.
• Document in electronic health records per agency standards and update care plans.
