Job summary

Gentiva is seeking a compassionate Hospice Registered Nurse to lead patient-centered care in patients' homes and other settings. You will develop plans of care, perform assessments, and coordinate with an interdisciplinary team to ensure comfort and quality of life.

You will educate families, document care accurately, and participate in on‑call activities while upholding regulatory standards and best practices.

Qualifications

• Minimum of 1 year as an RN in hospice, palliative care, home health or related setting.
• CPR certification required; current nursing license in the state of employment.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, self-care, and end-of-life care.
