Job summary

Gentiva seeks a Hospice RN / RN Case Manager to lead patients and families through the hospice journey, delivering hands-on nursing care in home settings and other living environments. You will perform initial and ongoing assessments, develop care plans, coordinate with an interdisciplinary team, and provide emotional and educational support to patients and caregivers.

Qualifications

• Registered Nurse with current license in the state of employment.
• One year minimum RN experience; hospice or home health preferred.
• Ability to perform comprehensive patient assessments and educate families.

Responsibilities

• Perform comprehensive patient assessments and reassessments; provide education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care across interdisciplinary teams with physicians and other clinicians.
• Educate patients and caregivers on disease process, self-care, end-of-life care, medications, nutrition.
• Communicate changes in condition to appropriate providers; ensure timely interventions.
• Support transitions in levels of care and referrals; document records timely.
• Maintain effective relationships with IDG and involve team as needed.
• Participate in on-call and patient support activities with clinical competence.
