Gentiva seeks a compassionate Hospice RN / RN Case Manager to lead in-home patient care and family education. You will assess patients, implement skilled nursing interventions, and coordinate with an interdisciplinary team to ensure compassionate, high-quality hospice services.

Qualifications

• Knowledge of hospice care.
• Strong clinical assessment and care planning skills.
• Ability to coordinate across interdisciplinary teams.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and visit schedules.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
