Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to lead in‑home nursing care and guide patients and families through the hospice journey with dignity and support.

You will conduct comprehensive assessments, develop and update plans of care, coordinate with an interdisciplinary team, provide education, document care, and participate in on‑call activities to ensure high‑quality, patient‑centered care.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams with physicians and other providers.
• Educate patients and caregivers about disease, end-of-life care, medications and nutrition.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care and transitions between levels of care.
• Document timely clinical records including orders, progress notes and plans.
• Maintain relationships with IDG and involve the team as needed.
• Participate in on-call and patient support activities.
