Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home-based settings across Alabama. You will perform comprehensive assessments, develop and update plans of care, and coordinate with an interdisciplinary team to ensure compassionate end-of-life support for patients and families.

Ideal candidates hold a current RN license, at least one year of RN experience, and CPR certification. Hospice or palliative care experience is preferred but not required.

Formación

• Registered Nurse licensure in good standing.
• CPR certification required.
• Minimum of 1 year RN experience in acute care, oncology, long-term care, hospice or home health.

Responsabilidades

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education for patients, families and caregivers.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications and nutrition.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care with referrals and transitions between levels of care.
• Document timely clinical records and ensure regulatory compliance.
• Maintain relationships with IDG and involve team when needed.
• Participate in on-call and patient support activities.
