Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager in Mobile, AL to deliver personalized, compassionate care in patients’ homes and community settings. You will perform comprehensive assessments, develop plans of care, and guide families with dignity and support throughout the hospice journey.

You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure timely interventions and education for caregivers, fostering comfort at end of life.

Qualifications

• RN license valid in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.
• CPR certification required.
• Valid driver’s license and auto insurance.

Responsibilities

• Be primary contact for patients and families guiding hospice journey.
• Deliver and document skilled nursing care per Plan of Care at home or other settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Coordinate care with interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe patient care and reassurance.
• Communicate updates to the care team regarding patient condition.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives for consistent care.
• Explain hospice services, end-of-life processes, and symptom management.
