Job summary

Gentiva, a national hospice leader, is seeking a dedicated Hospice Registered Nurse to serve patients primarily in home settings across our Alabama service area. You will assess, plan, and deliver skilled nursing care, coordinating with physicians, social workers, chaplains, and CNAs to ensure compassionate, patient‑centered end‑of‑life care.

You will educate families and caregivers, monitor symptoms, and document care plans while complying with regulatory standards.

Qualifications

• RN required with active license in state of employment.
• Hospice or palliative care experience preferred.
• Current CPR certification.
• Auto insurance and valid driver’s license.

Responsibilities

• Lead patient and family through hospice journey; serve as primary contact.
• Deliver and document skilled nursing care per Plan of Care.
• Perform initial and ongoing patient assessments and adjust care.
• Collaborate with IDG to plan personalized care.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe patient care and reassurance.
• Communicate patient status changes to care team promptly.
• Maintain clinical documentation and comply with infection control.
