Job summary

Gentiva Hospice in Florence, AL is seeking a dedicated Registered Nurse (RN) to join our field-based hospice team. You will deliver care in patients' homes or approved settings, coordinate with an interdisciplinary group, and support families through life-limiting illness.

The role emphasizes compassionate, evidence-based nursing, accurate documentation, and education for caregivers, with a focus on comfort, dignity, and end-of-life planning. One year of RN experience is preferred.

Qualifications

• Education: Registered Nurse with current license in the state of employment.
• Experience: at least 1 year RN in hospice/home health or related fields.
• Hospice/palliative care experience preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice care.
• Deliver and document skilled nursing care per Plan of Care in home or approved settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Educate caregivers and provide bereavement support.
