Job summary

Qualifications include an active Alabama RN license, 1 year of RN experience (hospice/home health preferred), and CPR certification. You will educate caregivers, document care, and participate in on-call activities as needed.

Qualifications

• Must hold an Alabama RN license.
• Minimum 1 year RN experience; hospice/home health preferred.
• CPR certification required.

Responsibilities

• Be the primary contact for patients and families guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home environments.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to build/refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families; offer bereavement guidance.
• Educate caregivers on safe, confident home care and practical training.
• Communicate updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives for consistent, compassionate care.
• Discuss medications, hospice philosophy, symptom management, and end-of-life expectations with patients/families.
