Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to deliver and coordinate compassionate hospice care in patients’ homes and related settings in Alabama. You will perform comprehensive assessments, develop and adjust plans of care, and collaborate with doctors, social workers, and other care team members to ensure high-quality, patient-centered care.

You will educate families, provide bereavement guidance, and ensure accurate documentation while navigating the unique needs of patients at

Qualifications

• Current license to practice nursing in the state of employment
• CPR certification
• Valid driver’s license and automobile insurance

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on Plan of Care in home or other settings.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with IDG to build and refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe care and comfort strategies.
• Communicate updates to care team for timely interventions.
• Maintain accurate clinical documentation and comply with infection control standards.
• Support quality improvement and scheduling initiatives.
• Navigate topics such as hospice services, symptom management, and end-of-life processes.
