Job summary

Live Well Homecare is seeking a Hospice Registered Nurse on call for after-hours triage and symptom management by phone, ensuring comfort and safety for patients at home. This role involves coordinating with the interdisciplinary team and updating the EMR.

You may also conduct occasional field visits to assess patient needs, guide families through end-of-life decisions with sensitivity, and provide compassionate support to patients and their loved ones in challenging times.

Qualifications
