Job summary

Advocate Health Care is seeking a Hospice RN to provide after-hours nursing coverage in home and community settings. The role includes comprehensive patient assessments, symptom management, and education to patients and families under hospice goals of care.

Responsibilities include triage, crisis support, and coordinating with physicians and the interdisciplinary team to ensure comfort and safety. A valid RN license, driver’s license, and BLS are required to deliver high-quality end-of-life care.

Qualifications

• Completion of an accredited nursing program.
• Active RN license in state of practice.
• Valid driver’s license.
• Basic Life Support (BLS) certification - American Heart Association

Responsibilities

• Provide comprehensive nursing assessment, triage, and intervention for hospice patients.
• Respond to calls to assess symptoms, educate, and determine interventions.
• Conduct in-person visits to manage symptoms and ensure comfort and safety.
• Manage end-of-life symptoms including pain, dyspnea, agitation, nausea, terminal secretions.
• Initiate physician orders and hospice protocols within scope.
• Provide emotional support and education to patients and caregivers.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Document encounters and handoffs in EMR timely.
