Job summary

Conemaugh Regional Hospice Johnstown, part of the Optum family, is seeking a Home and Community Based Services Aide to support a client at home with personal care and daily living tasks. The role emphasizes safety, confidentiality, and timely documentation using EVV systems.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation
• State-specific PA requirements for home health aide training
• CPR certification preferred or within 90 days of hire
• 6+ months of home care experience
• Ability to work flexible hours and independently

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
