Job summary

Heart N Home Hospice, part of LHC Group and a member of the Optum family, is hiring a Home and Community Based Services Aide to provide compassionate personal care for individuals who wish to remain safely at home. You will assist with daily living activities, meal preparation, errands and appointments, monitor health and vital signs, document care in the EVV system, and help keep a safe home environment.

Qualifications

Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
CPR certification preferred or able to complete within 90 days of hire.
6+ months of home care experience (preferred).
Ability to work flexible hours and independently.

Responsibilities

Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
