Job summary

Conemaugh Regional Hospice Johnstown, part of the UnitedHealth Group family, is hiring a Home and Community Based Services Aide to support individuals in their own homes. The role focuses on personal care, daily living activities, and coordinating with families to maintain independence.

You will work in a home-based setting, providing compassionate care, documentation, and safety monitoring while adhering to agency guidelines and state requirements.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation.
• PA state home health aide requirements met.
• CPR certification preferred.
• 6+ months home care experience preferred.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
