Job summary

Heart N Home Hospice, part of UnitedHealth Group, is hiring a home health aide to provide personal care and assist with daily living activities in clients' homes. You will monitor health, document vitals, and support clients with appointments while maintaining confidentiality and using EVV systems.

The role emphasizes safety, independent work, and compassionate care with a focus on diverse community needs and respectful treatment of all clients.

Qualifications

Current driver's license, vehicle insurance, and reliable transportation or access to public transport.
CPR certification or ability to complete within 90 days of hire.
6+ months of home care experience.
Ability to work flexible hours and independently.

Responsibilities

Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
Respect diverse backgrounds and provide care regardless of race, creed, color, sex, disability, or financial condition.
