Qualifications

Responsibilities

• Perform personal care tasks and attend to patient requests.
• Provide compassionate communication with patients and families.
• Ensure safety and proper use of equipment.
• Assist with bathing, dressing, feeding and daily living activities.
• Document care and report changes to RN Case Manager.
• Assist with mobility and transfers as directed.
