• Good communication with patients, families, and team.

Aufgaben

• Perform personal care tasks as assigned and promptly address patient requests.
• Communicate positively with patient, family, and staff while maintaining confidentiality.
• Ensure patient safety and use equipment correctly (foot stools, side rails, O2).
• Give baths, back rubs, oral hygiene, hair care, and change bed linens as directed.
• Assist with dressing, undressing, and feeding as assigned.
• Avoid homemade foods; follow dietary guidelines for patient care.
• Monitor skin condition and report pressure areas or changes.
• Report pain or symptoms to RN Case Manager; note caregiver concerns.
• Assist with ambulation and range of motion under supervision.
• Position and transfer patients safely; document any changes.
• Assist with self-administered medications as permitted by state rules.
• Provide bedpan/urinal assistance; help with light housekeeping tasks.
• Document care and observations as required; maintain records.
• Offer respite to the patient and family when on-site.
• Keep living area clean, organized, and safe.
• Contribute to quality improvement activities as assigned.
• Respect patient and family preferences and privacy.
• As a mandated reporter, escalate concerns per state requirements.
• Report suspected abuse or neglect to authorities as required.
