Responsibilities include bathing, mobility assistance, documentation in EMR, safety, and compassionate communication with patients and families, while following care plans and regulatory requirements.

Qualifications

• Certified Nursing Assistant credential in PA required.
• Ability to read and follow written instructions and document care.
• Comfort with hospice philosophy and terminally ill patients.
• Driver with insured vehicle and ability to travel as needed.
• Fully vaccinated per organization requirements.

Responsibilities

• Provide personal care and assist with daily activities as assigned.
• Communicate with patient and family using respectful, confidential care.
• Ensure safety and proper use of equipment and supplies.
• Assist with feeding, bathing, transfers, and activities of daily living.
• Document observations and report changes to RN Case Manager.
• Support in care plan adherence and quality/safety initiatives.
