Qualifikationen

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC)
• Current Healthcare Provider CPR certification (hands-on demonstration)
• Current Food Handlers Permit
• High school diploma or GED
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and provides specialized care to the terminally ill.
• 1 year hospice/long-term care experience preferred
• Basic computer skills to document in EMR
• At least 18 years of age
• Licensed driver with insured vehicle in good working order

Aufgaben

• Perform personal care tasks and respond to patient needs promptly.
• Communicate with patients and families with empathy and confidentiality.
• Ensure patient safety and proper use of equipment.
• Provide baths, grooming, oral care, hair care, and linen changes as assigned.
• Assist with dressing, feeding, and mobility as directed by the RN.
• Support medication reminders where allowed by state regulation.
• Monitor skin condition and report changes to RN Case Manager.
• Assist with range of motion and transfers as instructed.
• Provide respite to patient, family, or caregiver when on-site.
• Keep living area clean and organized and document observations.
• Adhere to care procedures and professional conduct standards.
• Participate in quality assessment and improvement activities.
• Report safety concerns and any suspected abuse or neglect per policy.
