Job summary

Apploi is seeking a hospice aide to work under an RN, performing personal care and documenting observations as per the patient’s plan of care. Responsibilities include assisting with bathing, mobility, feeding, and basic comfort measures to promote safety and quality of life.

Requirements include a state-approved CNA course, NAC, CPR, Food Handler’s permit, HS diploma or GED, and a valid driver’s license. Prior hospice/home health experience is preferred; EMR documentation is expected.

Qualifications

• Completion of a state-approved certified nursing assistant course.
• Current NAC certification.
• CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• HS diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and terminally ill care.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care according to patient plan of care.
• Provide compassionate communication with patient and family.
• Ensure safety and proper use of equipment.
• Assist with baths, transfers, and linen changes.
• Assist in dressing, undressing, and feeding as assigned.
• Avoid giving homemade foods; follow dietary guidelines.
• Monitor skin condition to prevent pressure injuries.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and prescribed exercises.
• Provide ROM and positioning as directed.
• Assist with reminders for self-administered meds where allowed.
• Assist with bedpans, urinals, and light laundry.
• Perform ROM and simple procedures within competency.
• Offer respite to patient/family when on-site.
• Keep living area clean and organized.
• Follow documentation and care procedures.
• Participate in QA and performance improvement activities.
• Respect patient/family environment and needs.
• Act as mandatory reporter for health dept requirements within 14 days.
• Report suspected abuse, neglect, or exploitation promptly.
