Apploi is seeking a Hospice Aide to work under RN supervision, delivering compassionate personal care to patients per each plan of care. You will document observations, report changes, and support safety and quality of life for patients in a home or facility setting.

Qualifications include a state-approved CNA/NAC, CPR, and high school diploma or GED. Prior hospice experience and basic EMR skills are preferred. This on-site role requires reliable transportation and at least 18 years old.

Qualificações

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and capable of providing specialized care to the terminally ill.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.

Responsabilidades

• Perform personal care tasks after patient assignment and notify Case Manager if unavailable.
• Provide patient and family with respectful communication and maintain confidentiality.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, oral hygiene, and bed linens.
• Assist in dressing, undressing, and feeding as assigned.
• Monitor skin integrity and report changes to RN Case Manager.
• Assist with ambulation, range of motion, and transfers.
• Provide reminders for self-administered meds as allowed by state regulations.
• Assist with light housekeeping tasks and laundry as needed.
• Report abuse or neglect per required procedures.
• Maintain clean and orderly living area and comply with care standards.
