Job summary

Apploi is seeking a Hospice Aide to support patients under the supervision of a registered nurse, assisting with personal care, mobility and comfort per the plan of care. You will report observations and care performed to the RN Case Manager to promote quality and safety.

The role emphasizes safety, respectful communication, and accurate EMR documentation, with required CNA/NAC and CPR credentials and a valid driver’s license where applicable.

Qualifications

• Completion of CNA or equivalent state-approved program.
• NAC certification is required.
• CPR certification current and hands-on (not only online).
• Food handlers permit valid.
• High school diploma or GED completed.
• Ability to read and follow written care plans.

Responsibilities

• Provide personal care and assist with bathing, dressing, and hygiene.
• Support in mobility, transfers, and ambulation as directed.
• Assist with feeding and monitor intake per care plan.
• Report pain or changes to RN Case Manager promptly.
• Maintain safe use of equipment and patient environment.
• Document observations and care provided in EMR.
• Communicate respectfully with patient, family, and team.
• Follow hospice philosophy and care standards.
• Provide respite and support to caregiver when on-site.
• Adhere to policies, procedures, and confidentiality.
