Job summary

Affinity Hospice Management LLC is seeking a NJ Certified Hospice Aide to work under the supervision of a registered nurse, performing assigned tasks per the patient’s plan of care to promote comfort and meet personal needs.

The aide will document observations and care performed and will be assigned to ensure quality, continuity, and safety of patient care. This role requires compassion, reliability, and adherence to regulatory guidelines.

Qualifications

• Must complete state-approved CNA training and hold current license.
• Ability to read and follow care plans and documentation.
• Basic computer literacy to document in EMR.
• Willingness to communicate with patients and families respectfully.

Responsibilities

• Perform personal care tasks as assigned and report to Case Manager if unable.
• Provide compassionate communication with patients and families while maintaining confidentiality.
• Ensure safety by using equipment correctly (foot stools, side rails, O2).
• Assist with bathing, grooming, oral hygiene, and linen changes.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding as directed; do not provide homemade foods.
• Monitor and report skin condition to prevent tissue breakdown.
• Report pain or changes in condition to RN Case Manager; consider caregiver dynamics.
• Assist in ambulation and exercises as instructed by nurse/therapist.
• Provide ROM and positioning as directed.
• Assist with self-administered meds as allowed by state rules.
• Assist with bedpans and light housekeeping as appropriate.
• Perform simple procedures with demonstrated competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly as assigned.
• Follow documentation and care procedures; maintain professional conduct.
• Participate in quality improvement activities as needed.
• Respect patient and family environment and personal needs.
• As a mandatory reporter, report non-compliance to Dept. of Health within 14 days.
• Report suspected abuse/neglect to appropriate authorities immediately.
