Job summary

Qualifications

• Completion of a state approved CNA course with current license.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and terminally ill care.
• 18 years of age or older and valid driver license

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide patient and family with confidential, respectful communication.
• Ensure safety and proper use of equipment (e.g., rails, O2).
• Assist with bathing, grooming, dressing, and feeding as assigned.
• Document care and observations in EMR and report changes to RN.
• Assist with mobility and transfers as directed.
