Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training
• Current Healthcare Provider CPR certification (hands-on demonstration required)
• Covid vaccination as per facility guidelines across multiple facilities
• High school diploma or GED
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized care to the terminally ill.
• 1 year of hospice/home health/long-term care experience preferred
• Basic computer skills to document in EMR
• At least 18 years of age
• Licensed driver with insured vehicle in good working order

Responsibilities

• Perform personal care tasks after patient assignment and respond promptly
• Communicate with patient and family with confidentiality and respect
• Ensure safety and use equipment properly (foot stools, rails, O2)
• Provide personal care: baths, back rubs, oral hygiene, shampoos, linen changes
• Assist in dressing and undressing
• Assist in feeding; avoid homemade foods
• Monitor skin and prevent tissue breakdown
• Report pain or changes to RN Case Manager
• Assist in ambulation and exercises as instructed
• Provide ROM and transfer/positioning as directed
• Assist with medication reminders as allowed
• Help with bedpans/urinals and light housekeeping
• Perform ROM and simple procedures as trained
• Provide respite for patient/family when on-site
• Keep living area clean and orderly
• Adhere to documentation and care procedures
• Participate in QA/QI teams
• Respect patient and family environment
• As mandatory reporter, report noncompliance to Dept of Health within 14 days
• Report suspected abuse/neglect immediately
