Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to work under the supervision of a registered nurse, delivering compassionate personal care per each patient’s plan of care. The role emphasizes safety, confidentiality, and timely documentation of observations and tasks performed.

Responsibilities include assisting with activities of daily living, mobility, hygiene, feeding support, and basic medication reminders within state guidelines.

Qualifications

• Completion of a state-approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on)
• Vaccination against covid per facility policy and guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Comfortable providing specialized hospice care to the terminally ill.
• Prior hospice/long-term care experience preferred; 1 year preferred.
• Basic computer skills to document in EMR.
• Must be at least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks as assigned and promptly attend to patient requests.
• Maintain positive communication with patient and family, upholding confidentiality.
• Ensure safety and proper use of equipment (foot stools, side rails, O2, etc.).
• Provide baths, back rubs, oral hygiene, shampoos and change bed linens as assigned.
• Assist in dressing and undressing patients as directed.
• Assist with feeding; avoid homemade foods prepared by the caregiver.
• Monitor skin and prevent tissue breakdown over bony prominences.
• Report pain or changes to the RN Case Manager; consider caregiver dynamics.
• Assist in ambulation and prescribed exercises.
• Assist with range of motion and transfers as directed.
• Assist with self-administered medications as allowed by state regulations.
• Provide bedpan/urinal assistance and light housekeeping tasks.
• Document care with demonstrated competency as ordered.
• Offer respite to patient/family/caregiver when on-site, as appropriate.
• Keep living area clean and orderly and adhere to care procedures.
• Participate in QA/PI activities.
• Respect patient and family environment and needs.
