Job summary

Affinity Health Management is seeking a New Jersey-licensed Hospice Aide (HHA) to support nursing staff under the supervision of an RN. The role focuses on patient comfort, personal care, and adherence to the patient’s plan of care, with timely reporting of observations and care performed.

Duties include bathing, dressing, feeding assistance, safety and measurements, documentation, and coordination with family and care team.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and terminal care.

Responsibilities

• Perform personal care as assigned, promptly addressing resident requests.
• Provide patient and family with respectful communication, confidentiality.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, oral hygiene, and changing bed linens.
• Assist in dressing, feeding, and mobility as directed by the RN.
• Record observations and report changes to the RN Case Manager.
• Assist with ambulation, range of motion, and repositioning.
• Assist with light housekeeping and linen tasks as assigned.
• Adhere to care procedures and participate in quality improvement activities.
