Job summary

Affinity Hospice Management LLC is seeking a NJ Certified Hospice Aide to support patient comfort under the supervision of a registered nurse. Duties include assisting with personal care, observation reporting, and ensuring safety in daily activities.

The role emphasizes clear communication with patients and families and strict adherence to care plans. The ideal candidate holds a state-approved CNA license, is at least 18, and maintains reliable transportation.

Qualifications

State-approved certified nursing assistant course with current license.
High school diploma or GED.
Ability to read and follow written instructions and document care.

Responsibilities

Provide personal care and attend to patient requests promptly.
Communicate with patients and families respectfully and confidentially, including confidentiality.
Meet safety needs and use equipment properly (foot stools, side rails, O2, etc.).
Assist with bathing, back rubs, oral hygiene, shampoos, and changing bed linen as needed.
Assist in dressing and undressing patients as assigned.
Assist in feeding the patient as assigned and follow state regulations on food safety.
Document observations and care performed for RN Case Manager and care team.
Provide respite for patient, family, or caregiver when on-site, as appropriate.
