Job summary

Affinity Hospice Management LLC is seeking a NJ Certified Hospice Aide to provide compassionate personal care under RN supervision, following the patient’s plan of care to promote comfort and safety. The aide reports observations and documents care performed, ensuring quality, continuity and safety in the patient’s living environment.

The ideal candidate holds a valid HHA license, strong communication skills, and the ability to work with patients, families and the care team.

Qualifications

• Completion of state approved CNA training and current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Communicate with patient and family using respectful, confidential care.
• Follow safety practices and use equipment properly.
• Provide baths, grooming, oral hygiene, and linen changes as assigned.
• Assist with dressing, feeding, and mobility as directed by RN.
• Document observations and care provided in EMR.
• Report changes in patient condition to RN Case Manager.
