Job summary

Harmony Hospice, LLC is seeking a compassionate hospice aide to provide personal care under the direction of a registered nurse. Responsibilities include assisting with bathing, dressing, toileting, feeding, documenting care, reporting deviations, using durable medical equipment safely, and taking vital signs as outlined in the Plan of Care.

You will support team care and participate in QAPI and IDG meetings as appropriate.

Qualifications

• Must have 75 hours of classroom training with 16 hours supervised practical training.
• At least 1 year experience as CNA/hospice/home health aide; hospice preferred.
• Must be 18 years or older and have a valid driver with insured car.
• Ability to communicate well both verbally and in writing.

Responsibilities

• Provide personal care (bathing, dressing, toileting, feeding) per Plan of Care.
• Document care provided according to agency policy.
• Report deviations from Plan of Care to Case Manager.
• Use Durable Medical Equipment safely and take vital signs when on Plan of Care.
• Foster cooperation among staff and participate in team and staff conferences.
• Assist with ambulation, transfers, ROM if documented.
• Provide respite for patient’s family/caregiver when on-site.
• Participate in QAPI activities and IDG meetings as appropriate.
