Job summary

Affinity Health Management is seeking a hospice aide to work under supervision of a registered nurse and perform care tasks per each patient's plan of care to promote comfort and meet personal needs. The aide reports observations and care performed to support quality, continuity and safety of patient care.

Responsibilities include assisting with bathing, dressing, feeding, transfers, and basic patient hygiene, while maintaining confidentiality and documenting changes for the RN Case Manager.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on preferred).
• Current Food Handlers Permit.
• High school diploma or GED.

Responsibilities

• Perform personal care tasks per patient plan of care and report to Case Manager if unable to perform.
• Provide effective communication with patient and family while maintaining confidentiality.
• Ensure safety and proper use of equipment (foot stools, side rails, O2, etc.).
• Assist with baths, back rubs, oral hygiene, shampoos, and changing bed linens as assigned.
• Assist with dressing and undressing patients as assigned.
• Assist in feeding; avoid homemade foods prepared by the aide.
• Monitor skin condition to prevent pressure injuries and document observations.
• Report changes in patient condition or pain to the RN Case Manager.
• Assist in ambulation and prescribed range-of-motion exercises.
• Support transfers/positioning and provide normal ROM as directed.
• Offer reminders for self-administered medications when permitted by state law.
• Assist with bedpans and urinals; light housekeeping duties as assigned.
• Demonstrate competency in range-of-motion and simple procedures as ordered.
• Provide respite for patient/family/caregiver on site when appropriate.
• Keep living area clean and orderly per procedures and standards.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and personal needs.
• As a mandatory reporter, report non-compliance with Dept. of Health within 14 days using required forms.
• Report suspected abuse, neglect, or exploitation to proper authorities immediately.
