Job summary

Shalom Hospice is seeking a compassionate Hospice Aide to work under the supervision of a registered nurse. You will perform personal care and comfort measures per each patient’s plan of care, documenting observations, and ensuring safety and quality of care in a home-like setting.

The role emphasizes respectful communication with patients and families, accurate documentation, and adherence to state regulations for medications and care tasks.

Qualifications

• Completion of a state approved CNA course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstrated).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfort providing care to terminally ill.
• Prior 1 year home health/hospice experience preferred.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family respectfully and maintain confidentiality.
• Ensure safety and proper use of equipment.
• Assist with bathing, oral hygiene, hair care, and bed linens.
• Assist in dressing, undressing, and feeding as assigned.
• Support self-administered meds as allowed by state regulations (reminders).
• Observe skin condition and report changes to RN Case Manager.
• Assist with ambulation, ROM, transfers, and positioning.
• Provide respite for patient, family, or caregiver when on-site.
• Maintain clean, orderly living area and document care procedures.
• Report any signs of pain or changes in condition to supervisor.
