Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to provide compassionate in-home or facility-based care under an RN's supervision. You will help with personal care, grooming, feeding, and mobility, while ensuring comfort and safety for terminally ill patients.

Responsibilities include monitoring skin integrity, reporting changes to the RN Case Manager, and documenting care in the EMR. A CNA/NAC certification and CPR are required; experience in hospice or long-term care is

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Hospice philosophy understanding and comfort with terminally ill patients.
• Prior hospice/home health/long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Provide personal care (baths, grooming, oral hygiene, bed linen changes).
• Assist in dressing, feeding, and mobility as assigned.
• Monitor skin condition and prevent tissue breakdown.
• Report pain or changes to RN Case Manager; maintain confidentiality.
• Assist with ambulation, ROM, and transfers.
• Provide reminders for self-administered medications where allowed by state law.
• Assist with light housekeeping and laundry.
• Document observations and care in EMR.
