Qualifications

• Completion of a state approved CNA course or equivalent training with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and can provide care to terminally ill patients.
• 1 year home health/hospice experience preferred.

Responsibilities

• Perform personal care needs and attend to patient requests promptly.
• Provide compassionate communication with patient and family, maintaining confidentiality.
• Ensure safety and proper use of equipment (e.g., foot stools, side rails, O2).
• Assist with baths, transfers, positioning, and grooming.
• Assist with feeding as allowed; avoid homemade foods.
• Observe and report changes in patient's condition to RN Case Manager.
• Support ambulation and range of motion as instructed by nursing staff.
• Maintain clean, orderly patient living area and perform light homemaker tasks.
• Follow documentation procedures and participate in QA/QI activities.
