Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our compassionate team in New Jersey. The role involves assisting residents in homes and facilities with daily activities, mobility, personal care, and medication reminders under the supervision of a Hospice Nurse.

Formación

• HHA license in NJ required.
• Understand hospice philosophy and care principles.
• Minimum of 1 year experience in hospice care.
• Reliable transportation to patients' homes and facilities.

Responsabilidades

• Assist with prescribed exercises and ROM under supervision of Hospice Nurse.
• Provide or assist with toileting, transfer, positioning and turning of residents/patients.
• Assist with ADLs including personal hygiene, grooming, mouth care, skin care and nails.
• Assist or remind patients to take medications per regulations.
• Help with light housekeeping of the patient’s unit or room and laundry as needed.
• Feed and assist with meals.
• Take vital signs (temperature, pulse, respiration) as supervised.
• Report changes in patient condition to the Hospice Nurse and facility nurse, documenting date/time and recipients.
