Responsibilities

• Perform personal care tasks as assigned and attend to patient requests promptly.
• Provide patient and family with supportive communication and maintain confidentiality.
• Monitor safety and use equipment properly; assist with transfers and positioning as directed.
• Assist with feeding, grooming, and basic care activities; report changes to RN Case Manager.
