Job summary

Affinity Hospice Management LLC is seeking a dedicated Hospice Aide with an active NJ HHA license to work under RN supervision. You will perform personal care, promote comfort, and document care provided to patients per their plan of care.

Responsibilities include assisting with activities of daily living, ensuring safety, and communicating effectively with patients and families. A valid driver’s license and caregiving experience in hospice or home health are preferred.

Qualifications

• Must have a state-approved Certified Home Health Aide (HHA) license.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and comfortable caring for terminally ill.
• At least 18 years of age and valid driver with insured vehicle.

Responsibilities

• Perform personal care tasks and respond promptly to patient requests.
• Maintain patient safety and use equipment properly.
• Assist with bathing, grooming, oral hygiene, and linen changes.
• Assist with feeding and remind patient to take medications as allowed by state regulations.
• Monitor skin condition and report changes to RN Case Manager.
• Assist in ambulation and transfers as instructed by nurse or therapist.
• Provide rest and respite for family/caregiver when on-site.
• Maintain clean, orderly living area and adhere to care procedures.
• Respect patient and family environment and confidentiality.
• Report any abuse, neglect, or safety concerns to supervisor per policy.
• Support documentation in electronic records as required.
• Participate in quality improvement activities as needed.
• Adhere to nursing scope limitations and do not perform nursing tasks.
• Assist with light homemaker tasks and transportation as needed.
• Ensure proper communication with care team and family.
• Adhere to agency policies and professional conduct.
• Maintain professional and ethical standards.
