Affinity Health Management is seeking a Hospice Aide (HHA) in New Jersey to work under an RN, performing tasks per the patient’s plan of care to promote comfort. The role includes documenting observations, assisting with personal care, and supporting family communication.

The aide will work to promote quality, continuity and safety of patient care, with duties spanning from mobility assistance to observation and reporting changes to the care team.

• Performs patient care tasks under RN supervision
• Communicates observations and care reports
• Adheres to patient plan of care and safety standards

Responsabilidades

• Assist with personal care needs and daily living activities
• Provide patient and family with respectful, confidential communication
• Ensure safety and proper use of equipment and supplies
• Assist with bathing, grooming, feeding, and mobility
• Monitor skin condition and report changes to RN Case Manager
• Document care provided and observations per policy
• Support medication reminders as allowed by state regulations
• Maintain clean, safe living environment for patients
