Job summary

Affinity Health Management is seeking a compassionate Hospice Aide to work under the supervision of a registered nurse. The role involves assisting with personal care, documenting observations, promoting comfort, and reporting changes in patient conditions per the plan of care.

Responsibilities include assisting with bathing, dressing, feeding, ROM, transfers, safety measures, and communicating with patients and families respectfully.

Qualifications

• Completion of a state-approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.

Responsibilities

• Assist with personal care tasks and report to Case Manager if unable to perform.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety; assist with ambulation, transfers, and range of motion.
• Assist with baths, dressing, feeding, oral care, and linen changes.
• Assist with light housekeeping duties and patient comfort measures.
• Report pain or changes in patient condition to RN Case Manager.
