Affinity Health Management in Indiana seeks a hospice aide to support patient care under RN supervision. You will document observations, promote comfort, and carry out assigned tasks in accordance with each patient’s plan of care, ensuring quality and safety.

Responsibilities include personal care, mobility assistance, communication with patients and families, and reporting changes to the Case Manager. You will work in a team focused on compassionate end-of-life care.

Competenze

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certification (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstrated).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Mansioni

• Provide personal care including baths, grooming, oral hygiene, and bed linen changes as assigned.
• Assist with dressing, feeding and mobility as directed by the RN/Case Manager.
• Report patient changes, pain, or concerns promptly to the RN Case Manager.
• Maintain safety and observe skin integrity; use equipment properly.
• Assist with self-administered medications as allowed by state regs (reminders).
• Keep living area clean and orderly; assist with light housekeeping tasks.
• Document observations and care provided in EMR per organization procedures.
• Participate in quality assessment and continuous improvement activities.
