Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstrated).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and comfort in providing specialized care.
• Prior hospice/long-term care experience preferred (1 year).
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured automobile in good working order.

Responsibilities

• Perform personal care tasks and attend to patient needs per care plan.
• Provide compassionate communication with patients and families.
• Maintain safety and use equipment properly (foot stools, side rails, O2, etc.).
• Assist with baths, grooming, oral hygiene, and changing bed linens.
• Assist in dressing, feeding, and mobility as assigned.
• Provide self-administered medication reminders as allowed by state rules.
• Report changes in condition to RN Case Manager promptly.
• Assist with light housekeeping and laundry as needed.
• Support transfers and range of motion as directed.
• Maintain clean, orderly living area and document care given.
