Ohio Living is seeking a dedicated Hospice Aide to join our team, under supervision of an RN, to enhance comfort and dignity at lifes end. You will perform personal care, observe and report patient status, and support families with empathy and respect.

The role requires a current Ohio CNA, valid driver's license, and prior home health or hospice experience is preferred. You may work weekends, evenings, and holidays as scheduled, contributing to compassionate end-of-life care.

Formación

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry required.
• Two years experience working as an CNA, home health aide, or other similar position in an acute care setting preferred.
• Must be able to read, write, speak, and understand English.
• Must possess a valid drivers license and acceptable driving record.

Responsabilidades

• Care Management - provides personal care as directed by the hospice nurse and follows the plan of care.
• Assists with mobility, transfers, toileting, hygiene, and activities of daily living.
• Provides psychosocial support to patient and family related to end-of-life care.
• Reports changes in patient condition to the RN Case Manager and documents observations.
• Maintains a safe and clean environment and supports homemaking tasks as directed.
• Drives patients to appointments and participates in on-call, weekend, and holiday rotations.
