Qualifications

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry required; or completion of Medicare competency evaluation program for home health aides.
• One year experience as a home health aide preferred.
• Must read, write, speak, and understand English.
• Must possess a valid driver's license and acceptable driving record.
• Must be insurable under professional liability and crime coverage policies.

Responsibilities

• Provide psychosocial support and personal care (bathing, oral hygiene, skin, nails, dressing).
• Assist with mobility and transfers including the use of transfer devices.
• Assist with prescribed exercises and toileting needs.
• Plan, shop, prepare, or assist with nutritious meals and prescribed diets; assist with kitchen cleanup.
• Assist patients to self-administer medications with reminders.
• Provide homemaking services and accompany to medical appointments as specified in the care plan.
• Observe and document patient status in EMR.
• All other duties as assigned.

Education

High school diploma or equivalent

Certified Nursing Assistant (CNA) in good standing

Medicare competency evaluation program completed
