Job summary

Luminary Hospice, LLC is seeking a dedicated hospice aide to join our patient-focused team in Ohio. Under RN supervision, you will perform a comprehensive range of personal care tasks to promote comfort and dignity for patients and families during the end-of-life journey.

You will observe, document, and communicate patient changes, while collaborating with Case Managers to ensure quality care. The role requires compassionate care, CPR certification, and a valid driver’s license in a patient home

Qualifications

• Active STNA or CNA certification required in the state of duties.
• Completion of state training requirements.
• 75 hours of classroom plus 16 hours of supervised practical training.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminal care.
• Satisfactory references from previous employers.
• Self-directed with minimal supervision and ability to cope with death/dying.
• Empathy for the needs of the ill and care recipients.
• CPR certification must be current.
• Tact, patience and good personal hygiene.
• Valid driver's license with insured vehicle.
• Probationary period to document competence.

Responsibilities

• Perform personal care tasks and promptly attend to patient requests.
• Provide positive, confidential communication with patients and families.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, and linen changes.
• Assist in dressing and undressing patients as assigned.
• Plan and prepare nutritious meals or assist with feeding.
• Take and record vital signs as ordered.
• Monitor skin integrity and prevent tissue breakdown.
• Observe and report patient condition changes to Case Manager.
• Assist in ambulation and positioning as directed.
• Provide medication reminders as allowed by state law.
• Maintain patient living area clean and orderly.
• Adhere to documentation and care procedures and professional conduct.
• Participate in quality improvement activities.
• Respect patient and family environments.
• Perform additional tasks as assigned.
• Provide respite when on-site, as appropriate.
• Assist with self-administered medications as allowed.
• Provide light laundry support.
