Qualifications

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry.
• Must read, write, speak, and understand English.

Responsibilities

• Provides personal care to patients as directed by the hospice nurse.
• Reports changes in patient condition to the RN Case Manager.
• Documents care performed in the patient’s medical record.
