Job summary

Crossbridge Hospice in Akron, OH is seeking a compassionate Hospice Aide to provide in-home personal care and supportive services to patients at end of life. You will work as part of a multidisciplinary team to honor the patient's Plan of Care. The role emphasizes dignity, comfort, and quality of life, requiring reliable transportation and a valid driver’s license.

A CNA certification (STNA in Ohio) is required, with high school diploma or GED preferred. Experience in home care is a plus.

Qualifications

Must be a certified nurse's aide (CNA).
State Tested Nursing Assistant (STNA) certification for Ohio.
Must maintain a valid driver's license and a reliable automobile.
High school diploma or GED preferred.
Demonstrate ability to work within a multi-disciplinary team; field experience preferred.

Responsibilities

Provide compassionate personal care and supportive services to patients in their place of residence in alignment with the Plan of Care.
Assist patients with tasks that support dignity, comfort, and quality of life.
Collaborate with the care team to implement the Plan of Care and updates during visits.
Ensure care aligns with the 4Ms framework (What Matters, Medications, Mentation, Mobility) and Age-Friendly practices.
