Job summary

Optimal Care in Ohio is seeking a Hospice Aide/Homemaker to provide compassionate, in-home care that preserves dignity and quality of life for patients at the end of life. You will assist with personal care, mobility, meals, and daily tasks while collaborating with nurses and the care team.

This role requires reliable transportation, state CNA certification, and willingness to travel within the Toledo/Lucas/Wood/Ottawa/Sandusky service area.

Qualifications

• Must hold current CNA certification and be in good standing on state registry.
• Completed a minimum of 75 hours classroom and supervised training with 16 hours practical.
• Reliable transportation and valid driver's license required.
• Experience as Hospice Aide/Home Health Aide is preferred.

Responsibilities

• Provide hands-on personal care (bathing, dressing, grooming, toileting).
• Assist with mobility, meals, and activities of daily living in patients' homes.
• Collaborate with nurses and care team to support patient needs.
• Travel within service area to patients' homes, facilities, or nursing homes.
