Affinity Care of Ohio is seeking a Hospice Aide to support patient comfort and personal care under RN supervision. The role emphasizes reporting observations, adhering to care plans, and maintaining safety and confidentiality in a hospice setting.

Applicants should have a state-approved CNA (NAC) and CPR, a high school diploma or GED, and basic EMR documentation skills. Hospice/home health experience is preferred, with a valid Food Handler Permit and driver’s license often required.

Qualificações

• State-approved CNA course completed or equivalent training.
• Licensed Nursing Assistant Certification (NAC).
• Current Healthcare Provider CPR certification (hands-on).
• Current Food Handler Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and care for terminally ill.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsabilidades

• Provide personal care and assist with activities of daily living as assigned.
• Communicate with patients and families respectfully, maintaining confidentiality.
• Ensure safety and proper operation of equipment (stools, rails, O2).
• Assist with baths, grooming, oral hygiene, and linen changes.
• Help with dressing, feeding, and perineal care as directed.
• Report pain or changes to RN Case Manager promptly.
• Assist with ambulation, transfers, and range of motion as instructed.
• Support with light housekeeping and laundry as needed.
• Adhere to care plans, documentation standards, and professional conduct.
